YOUR WINNING HAND
Provide us with the following documents:
• OSHA training documentation
• Immunization form signed by your physician, including:
1. TB (PPD) test (within the past year)
2. Physical
3. Measles, Mumps, Rubella (MMR) vaccine or exposure
4. Chicken Pox (varicella) exposure or vaccine
5. Tetanus shot (within the past 10 years)
6. Hepatitis B vaccine or titer "Updated in past twelve months (OSHA training, TB test and Physical)
• Signed release of information form enabling us to do a criminal background check
• Reviewed and signed HIPAA security training form
Dental Assistants:
• DANB certificate or Copy of CDA/RDA card
OR
• Certificate of graduation from an accredited dental assisting program (program accredited by ADA) or foreign dental school
OR
• Radiology certification AND 3500 hours of dental assisting experience (ie: 2 years full time or 4 years part time)
Dental Hygienists:
• Local Anesthesia Permit (if applicable)
